Referral/Intake Process
Families may be referred to FGDM by regional children social workers, emergency response workers, adoption workers, hot line workers, court services personnel, and family advocates.
The referring party will discuss FGDM referral with family or emancipating youth, and determine if either family or emancipating youth wish to have FGDM voluntary service. If family or emancipating youth wish to have FGDM service, then the referring party will complete the appropriate referral form.

The referrals are generated when the assigned regional children social worker:
Is open to accepting the family’s plan if the plan addresses the department's concerns and keeps the children safe.
Inquiries
For any and all inquires and to ensure that your Family conference or Emancipation conference referral has been received and is being assigned, you can contact your Spa FGDM Coordinator. If you do not know who your Spa FGDM Coordinator is, please refer to FGDM Administrator Phone numbers or FGDM Program Roster.
Upon receipt of Family conference or Emancipation conference referral by FGDM assignment desk, a referral will be assigned to a Spa FGDM unit in approximately five to seven working days (may vary depending of volume of referrals). Once a referral is assigned to a FGDM Spa unit, then assignment to a Spa Coordinator will take approximately three to five working days.
